A48-year-oldmanattendedtheoutpatientclinicformuscularweaknessandrestrictedabilitytowalk,whichhadslowlyincreasedsinceinfancy.Medicalhistoryrevealedmuscleweaknessnoticeablesincebirth,butnormalcognitivedevelopment.Hefirstwalkedatage18months,butalwayshadawaddlinggaitandhyperlordosis.Thepatientwasexaminedat3neuromuscularcenters.Electromyography(EMG),creatinephosphokinaselevels,muscletomodensitometry,and3musclebiopsieswerenormal.Resultsforacetylcholinesteraseantibodieswerenegative.Atage48years,thepatienthadincreasingaxialandproximalweakness,hyperlordosis(Figure,A),anddifficultiesrisingfromthefloor.Hewasonlyabletowalkmina6-minutewalkingtest.Peripheralfacialweaknessandbilateraleyelidptosiswereprominent.Clinicalhistoryalsoincludedasthmawithseveralepisodesofde
